Our Mission

Eldercare Referral Experts is led by a Bachelor-prepared Registered Nurse (BSN) with over 27 years of experience in home health and senior care, and is currently a Family Nurse Practitioner (FNP) graduate-level training.

With nearly three decades of hands-on experience caring for older adults, our founder has worked directly with:

  • Seniors living at home

  • Families navigating assisted living and memory care

  • Patients with complex medical and functional needs

  • Care transitions from hospital to home or facility care

This clinical background allows us to evaluate care needs beyond surface-level placement — focusing on safety, medical complexity, quality of life, and long-term planning.
